Whether you are a new patient or an existing patient, there are forms that need to be completed prior to your scheduled appointment time. Printing them, filling them out and bringing them with you will allow us to attend to your dental needs quickly. If you are unable to print these forms, please arrive at least 15 minutes prior to your appointment time. Forms can be found here.
For your convenience we accept Visa and MasterCard along with cash, personal checks, money orders and Care Credit.
Payment is expected when treatment is rendered. Please inquire and we will be glad to provide you with an estimated cost for scheduled procedures when we reserve your appointment.
We do NOT accept or take any insurance. We are OUT-OF-NETWORK or NON-PARTICIPATING. If you have dental insurance that allows out-of-network coverage, as a courtesy, we will submit dental claims for you.

We understand that dental emergencies do not always occur at convenient times and that our treatment schedule may be full. However, in the event of an emergency, our goal is to provide same-day emergency treatment. If the emergency is not during regular office hours, please call 315-376-3121. No-Show, Cancellation & Late Arrival Policy
Please give at least 24 hours’ notice for cancellations to avoid a $50 non-refundable/non-transferable fee (not covered by insurance). Allow extra travel time to ensure you arrive promptly for your scheduled appointment as if you are more than 15 minutes late, your appointment may need to be rescheduled and the cancellation fee will be applied. Please leave us a voicemail on the answering service if we are unable to take your call or the office is closed.
